Apart from the standard scenes in Death Note and Code Geass, there is: Black Lagoon(when levy goes crazy in the ship) Ergo Proxy(some scenes cant remember but the anime had this insanity vibe) Afro Samurai Btoom! Deadman Wonderland(several scenes) Evangelion Steins;Gate Canaan(somewhere in the end where a girl gets crazy and gets cut up) and last Kara no Kyoukai
